A shy bladder occurs when a donor is unable to provide a sufficient amount of a urine sample required for a drug test. When a shy bladder occurs, the donor is given a few attempts to be able to provide the required amount.

In DOT a shy bladder log is used to document the shy bladder process .The donor is advised to drink about up to 40 Ounces of water. If the donor is unable to use the restroom at the moment, they are given up to 3 hours to be able to produce the urine.

A shy bladder log should always be used by collectors to record the procedure of the shy bladder. Here is where the next stages for DOT and NON-DOT testing differ. The Medical Review Officer (MRO) gets engaged for DOT testing and works with the Designated employee representative to have the donor undergo a medical examination to see if there is a medical reason for the shy bladder. This returns to the MRO and is recorded as a Refusal if there is no medical justification or the donor declines to undergo the medical examination.
